Orphan Drug IMP Supply: Small-Batch Clinical and Commercial Packaging

NextPharma supports orphan and rare disease programmes across the full product lifecycle, coordinated through our Göttingen Clinical Trial Supply (CTS) site. From small-batch IMP packaging and blinding for rare disease trials, through multi-market commercial packaging of cold-chain-sensitive products, to serialisation and QP release, we cover the clinical and commercial stages a rare disease product passes through. Batch sizes, processes and project management are built around orphan drug economics, not adapted from high-volume lines.

01

Why Orphan Drug Supply Is Different

Orphan products break standard CDMO models at both ends of the lifecycle. Four characteristics shape how we work:

  • Small, high-value batches: rare disease populations mean commercial batches of a few hundred to a few thousand units, where the loss of even a handful of units is material. Low-loss, agile processes are not optional.

  • Global regulatory complexity: a single lot can split into 20 to 30 country-specific SKUs across FDA, EMA, PMDA and other authorities, each with distinct labelling rules.

  • Compressed launch timelines: priority-review and fast-track status can shrink the approval-to-launch window to a few weeks, requiring pre-validated, ready-to-execute supply.

  • Cold chain and high potency: biologics and highly potent APIs require validated cold-chain handling and dedicated containment at every step.

02

Small-Batch Supply at Göttingen CTS

NextPharma’s Clinical Trial Supply operation at Göttingen supports rare disease products with flexibility built in from the first pack:

Packaging from a minimum of 10 packs, scaled to required quantities of few thousandsBlinding, randomisation-list creation and adaptation, and emergency-code management, coordinated with your IXRS/IRT

  • Multi-layer, multilingual, booklet and wrap-around labelling, with in-house artwork and translation control

  • Comparator sourcing where quota regulation or marketing-authorisation-holder restrictions make supply difficult

  • Packaging solutions decided case by case: manual, semi-automatic or fully automatic

03

Commercial Packaging and Supply

The same site scales the product into commercial supply without breaking continuity:

  • Handling of delicate and high-cost products, including cold-chain-sensitive biologics with short out-of-refrigeration windows

  • Late-stage customisation (“bright stock”): unlabelled, filled stock held in inventory and finished to a market-specific configuration once a firm order is confirmed, maximising flexibility and minimising waste against uncertain launch timelines

  • Management of complex, multi-node supply chains typical of rare disease products

  • Commercial serialisation to meet global track-and-trace requirements

  • Bulk, Primary and secondary packaging for all dosage forms produced within NextPharma, and secondary packaging for products manufactured elsewhere

04

Multi-Market Labelling and Artwork

A single low-volume batch may need to reach dozens of countries, each with its own label rules. We manage the full labelling burden so a launch is never held up by artwork:

  • 20 to 30 country-specific SKUs from one lot, controlled through a single artwork and content-management workflow

  • Certified translation for multilingual markets

  • Region-specific requirements handled correctly: EU “blue box” national data, patient information leaflets folded to the carton, Braille on the outer carton, and market-specific symbols and codes

  • Patient-centric design with high-contrast, readable text and clear pictograms for dosing and storage

05

Serialisation and Anti-Counterfeiting

Orphan drugs are among the highest-value medicines on the market and a target for diversion and counterfeiting. The pack is the first line of defence:

  • Serialisation aligned with global mandates, including US DSCSA and EU FMD

  • Aggregation across the packaging hierarchy: unit to carton, carton to case, case to pallet, with reporting to the relevant regulatory databases

  • Tamper-evident, covert and overt security features integrated into the pack

  • Secure exchange of serialisation data with sponsors and downstream partners

06

Göttingen CTS: Capacity and Compliance

Göttingen CTS is NextPharma’s primary Clinical Trial Supply site and the coordination point for orphan drug supply, clinical and commercial:

  • GMP-regulated packaging in dedicated areas, handled by a deployed specialist team

  • Clean-corridor facilities and flexible rooms for project-specific packaging set-ups

  • QP release for pharmaceuticals, biologicals, IMPs, ATMPs and medical devices, in compliance with GMP, GCP and Annex 16

  • QP declarations and third-party site auditing for non-EU-manufactured material imported into an EU trial

  • Supporting analytical and microbiological services, including a dedicated microbiology laboratory with over 40 years of experience

One Integrated Offer: Packaging, Labelling and QP Release

Across both clinical and commercial stages, packaging, labelling and QP release are scoped as a single project. Release stays aligned to the same timeline as the physical supply, from first clinical batch through commercial launch, with one project manager coordinating throughout and no change of partner across the lifecycle. For a full overview of our clinical trials services, including IMP manufacturing and comparator supply, see the service hub.
